(Kytner, Ryan and Solomon, 2006).

חַיִּים, בְּלִי לְהֵהָנוֹת;

בעיקר אני קורא את השני, Are You My Mother של אחת Alison Bechdel.

The Technologies of the Self, מ1988 ככה. בעצם יותר במובן של לדאוג לעצמך (take care of yourself) ברמה של well-being.

Stereo Mind

This morning I went to a meditation class. A very short one. The teacher told me to sit comfortably, breath, try to count my breaths from one to ten, then count again, and if I think of something, let it be and let it go.

I had trouble counting. I started, then it got boring so I thought between breaths, of this and that, and I tried to notice when I was holding on to a thought, like the teacher's instructions, or how I really want to stop thinking, etc.. So I started debating these thoughts with two voices in my head, and so I lost the counting voice. At first I kept saying, oh, I must be somewhere around 5, and started from there. But then I noticed that by guessing I was around 5, I was holding on to the thought that maybe I was at 4 or 7, so every time i wasn't sure, I just started over from 1. I did it for a while and then time finished.

Afterwards I thought this is exactly what I was curious about when I studied about ACT (Acceptance and Commitment Therapy), or when I read about The Capacity to be Alone, or what I understood from Yoda's instructions to be mindful of myself....

But I am writing this just to note in my personal growth log, that it seems I have a stereo mind. I am talking to myself. There are two time separated channels in conversation. The counting took over one of them and bored the other. The first voice is me, the second voice is my self (which is me in a moment's delay).

An Appreciation of Feminism to My Understanding of Otherness

Soon I will be 31, old, I have lived and loved, and I have just recently found out that I was mostly blind to world around me. It began with a broken heart, then there was a post I wrote about Jessica Benjamin's article, Recognition and Destruction, that made all my experiences in life so far collapse into perspective of intra-psychic experience, then there was a gender conflict group I instructed, then there were a few feminist blogs (1, 2 in Hebrew) I read recently that made AWFUL sense to me, and this post is just another attempt to regurgitate this idea in and out of my head... I will try to make it communicative, even if lengthy...

Benjamin describes the psychological development of the capacity to recognize other people as subjects, and not as objects. She writes as a feminist, relational psychoanalyst, translating the legacy of patriarchal psychoanalysis and object-relations (how babies see moms) into critical inter-subjective terms (how people see each other).

Now, I am sure there are intellectuals who do a better job of explaining intersubjectivity per se. Psychoanalysis is always a step behind philosophy, literature, and sociology too maybe, as it bothers with patients instead of introspection, and insists on rules instead of insights.

Also, this thing about the developmental metaphor, where psychoanalysts describe human relations as stemming from mom-kid relations is not always helpful. Yes, thinking of babies makes people less judgmental, and they can criticize social constructions this way. But this is just a rhetoric manipulation, and using the same metaphor all the time, claiming that it is the natural cause of how people grew up to be douches, is mere justification of things, or an anxious apology for thinking differently.

So, Bejamin has two very strong insights in this article, that rocked my world:

1) Objectification:
People can relate to each other as objects, or as subjects. To relate to a person as an object is to relate to a narrow aspect of that person, and deny other aspects. For example, when men treat women as sexual objects, not really caring other aspects, such as their intellectual traits. I think this is how the objectified side would experience objectification.

But Benjamin describes the psychology behind this act on the objectifying side, and helps shed light on other forms of objectification that are harder to notice. To do this, she refers to the psychoanalytic theory of object-relations. The basic idea is that people reduce reality into simple terms of good and bad. For example, when someone cuts you on the road and you curse his mother and wish him death, or when someone you like brings you coffee to bed and you wish it will always be like this and you are thinking for names for your prematurely fantasized kids. This is a dichotomous way to see the world as either completely persecuting or completely benevolent, and to see yourself as a 100% victim or 1000% wonderful.

This way, we see the world through a dichotomy of wishes and terrors. In order to see the person who cut you on the road as a persecutor, you must deny that you probably cut others all the time. In order to see that girl as the mother of your children, you really need to deny that you've been behaving like a lazy slug lately, and that she may want you to get out of bed and get a life.

So, objectification is not just the reduction of another person to a narrow aspect, such as a sexual fantasy. It is seeing another person as part of your internal reality. The reduction into a narrow aspect of that person is secondary, and happens because our internal reality is usually quite simplistic. But this allows us to examine two new forms of objectification: complex and non-sexual.

Complex objectification is something that may happen when a highly introspective person tries to fit reality into his (yes, mine..) internal schemes. After a few times of getting coffee to bed and not finding eternal love as a result, such an introspective person may learn that other people are not simple objects. A weaker mind may then be forced to give up this simplistic view of reality. But the introspective virtuoso, may create more complex objects, and try to force reality to fit them. For example, being equal and correct and making coffee too sometimes. But this is still just a way to maintain the other person as a 100% loving object. I now begin to think that there is something very controlling about making someone coffee.

It's more complex than forcing someone to make you coffee. But it's still a way to see someone as your fantasy, by compromising the fantasy a bit. It's also non-sexual. Feminists rightfully fight against sexual objectification as it relates closely to rape and harassment. But other forms of gender oppression, such as wishing someone would be your mom and bring you coffee to bed (now you get the fantasy?) work the same way. You still relate to a narrow aspect of another person.

1.5. Denying subjectivity
(Well this isn't something Benejamin said, it's more of a thought that helped me understand her.) Looking at the psychology of objectification on the objectifying side is very helpful. It helps distinguish objectification that a person performs willingly on him/her-self from objectification that serves someone else's wishes. When I say person I mean both subject and object. A person is an object as it has objective traits and functions, such as being tall or intelligent. A person is also a subject in the sense that a person can relate to its objective qualities, by judging itself, wishing to change, or just know itself. Here I really need to brush up on Kristeva's notion of subjectivity as a continuous process of relating to the formation of yourself as an object. But let's keep it simple, and say that a subject is a point of view, individual, a current reflective thought that is on the edge of your experience. For example, I am currently noticing that I am listening to the song Love is in the Air, and now I am noticing that it has something to do with my post, and now I am embarassed to think that my kitsch playlist has become public knowledge, and now I need to go the bathroom, and now I think of my previous post where I wrote about pee-anxiety, and so on and so forth. As you can see, I keep relating to myself and to the world, and this is my point of subjectivity.

To objectify a person is not only to reduce the person into one aspect, but also to deny the person's subjective point of view of him/herself. A few years ago I watched Israeli news after a bus bombing, and there was a recurring ritual of inviting an Arab Knesset Member for an interview and asking whether he denounces this act of terror or supports it. This time was like all others, only that I listened to what the Arab KM said. He said that the situation is unfortunate, that people are dying on both sides, that both sides are wishing for a change. The interviewer was discontent, insisting on a simple yes or no - do you denounce terror or not? In the national grief after a bombing, an Arab Knesset member friend or foe, and what how he relates to his being a foe, is irrelevant.

Recently I read a feminist post about consensual sex, listing the 10000 ways a woman can refuse, and 100 cases in which consent is invalid due to coercive dependency. I felt like this was a more politically correct way to insist on a simple yes or no, and denying the way a woman's subjectivity. Of course, there was a second post, for advanced readers, that discussed the dichotomy of the consent discourse, which recognized the need for a respectful dialogue, rather than an elaborate, nonetheless binary, yes/no algorithm.

But dichotomy is not all bad, or we would not agree to it, or do it to ourselves... I sometimes like it when I am perceived as an idealized,  all-good-object. It boosts my ego. To deny that people serve as sexual objects for each other would also be unrealistic. People have sexual desires, and people want to be desired sexually. People objectify themselves, when putting up a superficial persona trying to look cool, or when dressing up sexy.  This is a very crucial point for feminists then. I would have to guess that when a woman dresses up sexy, she does it for herself in a way. I mean, this is not an internalization of the male objectification of women as worth only as sex objects. It could be, but it could also be something more interesting.

It seems to me objectification got a bad reputation because of coercion. I would describe a neurotic person as superfluous with subjectivity, not willing to rest his point of view of himself, and just be as he is (yes, me). Letting go of this reflexive function of subjectivity is a goal in many religions, therapies etc.. Not to say that religions should be the benchmark of goodness, but that people do desire to stop desiring, to be still, content.  Self-objectification means enjoying the role I play for others.

2. Retaliatory VS mutual relationship
So we're back to Benjamin, and this is the real gold, so forgive me for not really having it down. She says that when we see someone else as exterior to our intra-psychic fantasies and terrors, we recognize his/her subjectivity. We listen to how that person sees him/her-self, and how he/she relates to him/her-self. Recognition is a key idea in relational psychoanalysis, so I will be very modest at trying to define it. I think it is giving validity to a person's relation to himself, to his subjectivity. For example, I could recognize that the person who cut me on the road did it because he is afraid to be late for work, or that 1000 other drivers cut him on the road, and he decided to do the same. But I would not really be recognizing anything when I am isolated in my own car. This would be similar to left-wing Israelis trying to figure out Palestinians without talking to them. You really need to roll down the window and talk. The girl who brought me coffee to bed, why did she do it? What did it mean to her?

Benjamin refers to Hegglian Master-Slave dialectics to explain this, and I really think you should read Heggel or Benjamin's explanation, but if you're lazy, I will give it a shot.  A person wishes to be recognized a point of subjectivity, that relates to the world. But the world as an object cannot give recognition. Only when you are met with another person's subjectivity you realize that you are a subject. When you see the depth and confusion, the way a person relates to itself, self-estranged, blind to itself, only then you understand that the mess you feel is what you are, and that you may also be blind to yourself.

Only a subject can recognize a subject. This leads to some sort of a loop: you use someone as a mirror, this is objectification, like asking for someone to tell you how great you are, But then, you realize that the mirror function doesn't work as a function, because in order to mirror a spontaneous source of reflexive desire, the other person must not obey predefined rules, like needing the compliment to come without you asking for it. So you have two subjects using each other, one on the expense of the other. This is a retaliatory relationship, where you make someone coffee so that they will make you coffee later, where you deny someone a compliment just so they will not take you for granted. This is similar to how Israelis drive, always one on the expense of the other. And the worst thing is to be taken advantage of. An eye for an eye, just not to have less eyes than the other guy.

Ben Kingsley's Gandhi movie was also influential for me, as I understood how Gandhi recognized what makes the British tick, that with all their violence they think they are civilized, and his non-violent resistence forced them to act in an uncivilized way and shoot innocents, which was a way to deny them recognition as good guys...

But then Benjamin describes a moment of mutual recognition tht happens just at the switch, when the user becomes the used, and vice versa. Like in WWF tag team matches, where one wrestler tags his buddy for a substitution, and for a minute they are in the ring together. Or like two drivers cutting each other and realizing that they know exactly why the other guy is behaving like a douche. That moment of mutual eye gouging when you see through another person's eyes. It's just a fading moment, that collapses very quickly into retaliatory relationship again. But this is the moment I have been searching for. It has to do with dialogue. Not being correct, or justified, or a nice guy, out of my own oedipal guilt or out of a narcissistic wish to be 1000% loved. It has to do with taking responsibility, owning my role to other people, being fair and recognizing my power, listening, putting my fears and wishes aside, thinking less, and the hardsest thing, as if those were not enough, is being here and now, in touch with how things really are and how they keep switching and nothing is definitive... I'd rather not be too definitive, as I won't be able to take responsibility for some sort of final take on things, so this maybe a verbose start of a dialogue I guess..

But I'm really hungry now, and I need to ask someone some questions about coffee and stuff....

I always think of intersubjectivity as mixing concentric Hora circles, here's something close...

PS:
Of course, there is something to gain from mutuality, other than moral peace of mind, which is kind of a retaliatory motivation... Instead of using the other to see yourself, you look at the other for him/her-self, and the more profound you look into the other, the more you can see yourself, in ways not limited by what you are afraid/wish to find out.

Notes on The Capacity to be Alone by DW Winnicott (1958)

Yesterday I read Winnicott's essay The Capacity to be Alone (1958, here in Hebrew), describing the ability of an individual to be quiet. I'll try to explain it here in my own vulgar terms.

So, as it is commonly known, the ego's function is to be the mediator of id drives and the environment's demands. For example, to want to go to the bathroom, but to understand that the bathroom is taken, and to keep typing for a bit longer until the need to pee can be met. I would describe the capacity to be alone as the ego's ability to rest from this mediation task. Winnicott gives the example of two lovers laying side by side after sexual climax. They are mutually fulfilled, not needing to satisfy themselves nor to satisfy the other, they can just wander in thoughts, resting in cool sweat, each to his (or her) own. They are together alone, their capacity to be alone comes from their fulfillment and exhaustion, of their need for each other, and for sexual gratification. This is how the ego rests.

Now forget about sex and think about a tiny little baby, laying, wrapped in a tight bundle. The internal drives of the id may be pre-verbal hunger. Try and imagine that hunger before you understand what hunger is (like in the film Wings of Desire, where an angel falls to the ground and feels hungry for the first time, and cannot describe it, though he knows how to speak). Imagine the helplessness when you are laying in your shit, and it stings and itches, and you don't know if it will ever stop. These sensations are exciting and overwhelming, drawing all the baby ego's attention to them. In adult terms, think that you are so anxious about eating or peeing that you have to do it all the time, just fearing that you will need it later and won't be able to, or that if you don't pee or eat, and you will have to sit in class or in a meeting, and be hungry or have to pee, it would be a stimulus too strong to ignore, because your ego is completely drawn, to your internal states and needs.

On the other hand, there's the environment, that keeps infringing on the ego with noise, demanding response to a smile, interrupting with untimely feeding. Winnicott does a great job in other essays (which I didn't REALLY read) in describing love and nurturing as something that should be balanced, somewhere between neglect and imposition. His popular term, the good-enough mother, refers to a caretaker that gives the right degree of space for the infant, to learn that he (or she!) will survive a minute of hunger, but not too much space, so the infant won't lose the association between his (or her) needs and their fulfillment (and then forever fall into dark loneliness, with needs that can never be satisfied, and love that can never be absorbed!! behhh!!!!). On the other side of that, relating to the infant should be balanced with the right degree of warmth and closeness, to comfort the infant, and give him (or her...) the basic feeling that the world is good and sweet, but not too much, not to interrupt him, with overbearing love, that impinges on his quiet, demanding him (or her...) to accept a nipple, or to be tossed around, when he (or she) is not ready for it (and then be coerced into compliance with the caregiver needs, losing touch with his (or her) own needs, creating a false persona that aims to please mom forever, getting good grades, eating everything off the plate, studying medicine and marrying a Jewish doctor, only to feel empty and void of personal meaning and vitality, as his (or her) needs were neglected a long way back, and can no longer be fulfilled by achievements that are meant to please the caregiver, and not the authentic self!!! ouch!).

So you have this baby, as I said, wrapped in a tight bundle, with his raging internal sensations, and his (or her) mom and dad fussing around him (or her), and in between his baby-ego is trying to make sense of things. What is going on? Is this gas? Is mom holding me too tight? What how why? And, probably more interesting to a tormented gassy babies, and later to accomplished adults: what can I do about it? Kicking, crying, smiling, blabbering, burping, maybe going to the library to sit quietly and getting some work done, or whatever is in the baby's power to do. I really wanted to get to the point of the bundle: it isolates the baby from the environment, and doesn't let it influence it by moving, forcing it to attend to its internal states and sensations. The bundled baby has a fresh diaper with some balm, and a full stomach after burping, and he (or she) is laying with no overwhelming internal sensations. No breeze, no fussing around him (or her). The emerging ego function learns what quiet is (This will take the anxious, over attentive ego years of meditation and/or good therapy to learn when he (or she) becomes an overworked desensitized self-estranged yuppie adult).

So let's summarize:
1) The ego is the mediator of internal and external states.
2) The ego should be taught to rest.
3) The ego can rest when it is well nurtured and fulfilled - in relation to internal and external sensations and demands.
4) This rest is a form of self-calibration, which allows later interactions with the environment or later internal needs to be fulfilled and experienced as relating to the self (not as false self that is attuned to the environment, nor as a desperate attempt to quiet overbearing internal urges). I didn't say this before, so it's good I summarized.

Here's Hedwig's song about the origin of love as a desperate chase of a completing other. Remember that Socrates describes the higher form of love as non functional, like the satisfied lovers in bed:

My comments and personal highlights are:
0.5) Baby mental life is a metaphor for me, I don't care for the pathogenesis...
1) Love can be an interruption.
2) To be alone and quiet, a person's needs must be nurtured and his (or her) relationship with others must be fulfilled. To be together and always missing each other, or frustrating yourself, is a form of loneliness.
3) Fulfillment is not objective, nor absolute. It's a sort of magic that happens when a deeper level of your existence is recognized. We do not chase fulfillment, that would be desperate if we are far from it, and depressive after we get it. We are in search of self actualization. To find a fitting challenge, to be recognized for strengths and shortcomings.
3.5) Exhaustion is another form of fulfillment that has to do with changing our expectations and meeting our limits. What makes exhaustion meaningful and not depressive? That it follows an authentic-self's wish.
3.75) It is only after we are quiet that we can feel ourselves as autonomous, not as vessels of desires, or as pleasing robots. If we are autonomous, then we are authentic, since we do what we want, we influence the world in our way, our relationships are deeper and more meaningful than functional relations, and what we have done, we have done ourselves. Not to know what quiet is, means that we are always driven by something else, from within or without. We may get everything we want, we may be loved by all, but we will not be satisfied, as our selves are missing in the middle.
4) The unbearable tension circumscribed here is between subject and object. The ego's ability to rest is our ability to be an object, subjectivity as the point of view to nothing, to be still and not to be depressed or desperate.

PS: if anything in my account of Winnicott is wrong, I'd appreciate a reference to what he really says, and a reference to someone who says what I said better, but no need for "oh this is inaccurate" coz I know this already...

Notes on Slavin's Explanation of Agency

"Whether I shall turn out to be the hero of my own life, or whether that station will be held by anybody else, these pages must show" (Dickens's Copperfield's first words).

I attended a guest lecture by Jonathan Slavin at Tel Hashomer military mental health clinic.  He presented a case study, there were a lot of free pastries and soft drinks, and then he spoke about the concept of Agency in relational psychoanalysis.

In the case  he presented, meetings were held via Skype, and included email correspondence. The main approach of the therapy was to allow the patient to recognize his parents as subjects, in order for them to be available to recognize him  as  a person in turn.

Slavin explained Agency for dummies, or maybe it sounded this way because he was aware of the audience not being native in English, but anyway, I found it very catching, it went something like this: "You   know what an agent is, like if you were an actor, and you have an agent, and his job is to go  out and get you acting jobs, that is what an agent does, like when you go out on a vacation and you get frustrated with too much information online so you go to an agent that arranges things for you. An agent is someone who acts in your behalf, who acts for you. Well in relational psychology,  we  mean that a person should be his own agent, to act for himself".

Well, it was most catching in my view not because I didn't understand that before in simple terms.  Instead, it was enlightening since I imagined the actor, sitting at home waiting for jobs, calling his agent,   begging for something good, that really seemed to me like intrapsychic isolation. Then the idea of an abusive agent, like friends' Joey and his agent, or many other TV portrayals of agents, made me realize how abuse and agency were linked - the actors ambitions to get famous and give it his heart used to stuff him in cheap nude scenes. Well maybe this is not so smart on my part, but I will try to compensate by saying it is quite the Lacanian view of trauma too.

Also,  Slavin made a short example of a mother-daughter interaction that  I later told my mother, and helped me explain to her what a few years of quoting Zizek and Winnicott did not do. Instead of telling the resisting kid after a minute of arguing: "you know you want to put your coat on and come with me  and pick up your brother", he suggested to say "I know you don't want to go, but we have to, so put your coat on and let's go pick up your brother". By that keeping the child's subjective autonomy in tact, frustrating her wish to stay and play, while fulfilling her need to be recognized as a subject. My mom, usually expressing much interest in my Zizekean comments, while guilting inside about not being enough good-enough, could very easily relate to that and say, "huh, that's good, not to take that from the child...".

Last, Slavin made a very brief prelude to the idea of recognition that  it was like a mother being highly attuned to the needs of the child, and how it forms the child's mind to realize his effect on the world. This reminded me of a research job I used to have. Very abusive in employment terms, yes, but very interesting in content. During my BA, I was a research coordinator in a joint research for Harvard Med School and Tel Aviv University, under the supervision of Prof. Als who leads a  research grounded approach  for developmental interventions with infants (NIDCAP). Her stuff is all fMRI validated, in a way that would make adult therapists very much ashamed.  Anyway she and her colleagues have a lot to  say about the infant's arousal levels, and how different  internal states co-regulate each other, and how the caretaker helps the child  regulate itself. For example, the psychomotor system regulates the attention , so when moving the infant is less attentive. In this way the mother can wrap the baby in a bundle, not letting it move, and enabling greater attention to internal states and interaction with the mother.  Now I am still bitter about the terms of my employment in that job, and I was going through a Kleinean thing maybe, but I now understand the link between holding, the formation of subjectivity and a sense of agency in infants a lot more.  About the lack of physiological correlates to adult psychotherapy concepts, I think I would like to explore a sense of agency as a regulator of arousal.  I think that is even very obvious to see, and must have been explored under other theories: that when your influence on the external situation is clear, arousal is regulated - either high when response is possible, or low when it isn't. But I make a mess of things.

Let's leave it with a mess of coordinates to figure out later.

Here is a picture of  Columbus. Did he get secret maps from ancient chinese explorers before him? Did the scout on his mast really expect to fall off the end of the world?  Anyway his quest was great.

PS:
I mentioned that he used Skype for remote therapy sessions. I listened to bits of them. I wondered if they could be recognized semantically, grammatically or structurally, to be more or less effective, or successful. Again my wish for objective validity. I guess it is a wish for recognition.

Client Relational Psychotherapy

Yesterday, interviewing for a possible internship, I had to excuse my zeal for results in psychotherapy. I did so apologetically,  and later I  discovered it was also unnecessary, as the interviewer just wanted to see that I am on the normal side of things. But here is  my apology as I phrased it:

First I explained the personal motives I have for pursuing such materialistically reductive method. I said that when I was a youth instructor , working with at-risk teenagers, I was very frustrated at team meetings. We would discuss a case, come to some psychodynamic insight about the teenager in question, and then ask what to do about what we understand. For example, if one of the girls woke up late for school, we would probably understand it either as fear of facing  a threatening world, or as enjoyment of a feeling of safety with the staff.   Both ways of understanding the behavior in by the emotions and perceptions behind it, would lead us to the following question: so what to do about it? Obviously one could go both ways in the first way of understanding the behavior - the staff could promote exposure to threats, demanding the teenager to cope with challenges,  enforcing the rules in order to promote a sense of  achievement over  avoidance.  The other way to react educationally was to talk about feelings, understand the threat in getting up for school, by that forgiving  punishment, helping the teenager untangle its defences  and providing support, facilitation, and mediating the challenge, to promote a feeling of safety and acceptance as the basis of future coping.
The other way of understanding the behavior, as a sense of safety with the staff, also leads to the same question - do we challenge safety or do we facilitate it?

The problem with these question is not that they are not solvable, they are very solvable. But it seems they are transparent in the psychodynamic discussion.  Anyone who ever discussed a case among colleagues knows that there are many approaches. Should we love the patient, or should we frustrate him? When to be Kohutian , when Lacanian?   Well in the team meetings I described these questions would be solved by staff policy (all girls must go to school or be punished) , the personal  taste of the director (Behaviorism) , and technical circumstances ( punishment bank is not yet depleted) .

Well one day I will write a post about abuse of therapeutic power, regarding that very institution,  but the point is, that these are peripheral considerations.  They have to do with  who's in charge, what are the resources and a lot of other factors that are not specific to the patient. The dynamic interpretation of the behavior is specific, although only hypothized. But it does not give a specific intervention for the patient.

My idea  was that the intervention should be adapted to the ego  strength of the teenager in question. As Winnicott puts it, when explaining the need for holding, "challange strengthens the strong ego, and weakens  the weak ego". And so I thought that one should evaluate the teenager's ego  strength , and facilitate/challenge by this rule. I think this is a key to comparative psychotherapy interventions and should be a leading factor in determining which intervention when and for whom.

However, the problem is that to offer my solution I should have been in power to call the shots. It would have been my personal Winnicottian taste.   Maybe I would have been right, maybe I would have been wrong,  who knows? As I would prefer to avoid abuse than to be right, I think there should be a way to say who is right.

If abuse is misuse of power,   for  any other means than to help the patient, then   we should be very much concerned with what helps the patient. The patient should be listened to. The experience of the patient in therapy becomes an ethical matter.  Yes, there are technical considerations, such as negative transference, where the patient better go through negative emotions toward therapy itself in order to have a corrective experience, not as bad as the damaging experience.  There are stuff the patient is no conscious of . But it is the patient that gives licence to  any negative experiences during therapy such as these.

This idea goes in line with research of the therapeutic alliance, and   the patient's early positive perception of the relationship with the therapist as a main predictor of therapy effect  in the view of the therapist and  the patient in later stages. Meaning, it is a technical consideration, not just a goal.

Research dealing with the threapeutic alliance calls the patient a client. As the relationship changes from that which a person has with a medical authority to that which is common to have with a  service provider, e.g.: "This is not what I'm paying you for, let's stop talking about my infancy and focus on my current anxiety attacks".

Let's face it briefly and get it over with: globalization, onlinization and other stuff that made many customer services  available , replaceable and  sometimes   marginal. Many customer services now focus less  on the   stuff they do or give to the customer, and more on how they make the customer feels about it. Customer becomes user in this world, not obligating to a provider, not needing anything really, just using and enjoying.  Businesses care less about the technical back office and how stuff works, even benefits to the client are secondary, what matters is what is experienced as desirable by the  client, now user. It is an age of user experience over all. Nobody cares how it works, as long as it pleases me, touches the things that are important to my identity, makes me feel good about myself and what I do.

I drifted some.  But I nearly explained the reason for the post title. Anyway, the interviewer asked me why I focus on results. I said that results  make sure that we focus on the patient experience (thinking to myself UX).  "How does it fit with relational psycholoty, saying that we have many many ways to relate to the patient, as he has may many ways to relate to us?" I was then asked. And that is where I get to my point.

I believe in psychodynamic understanding of people. Yes, theorists of many approaches appeal to
me with their insights on humanity. Some are easier for me to apply on myself and others, some I should learn more about to be able to use their methods effectively. I am a pscyhodynamic fan.  Exactly  because of  the richness in the ways I could understand myself and others, I find a great need for  measuring what is needed, what is right, and what works.

I think psychoanalytics , a results based data mining approach to psychotherapy, is in the way of a client centric psychology, that deals with what is important to the patient .  Our theories better be broad and rich as ever, we should try, using them as maps for exploration. But we should also work hard on measures for therapeutic experience, on what feels better, on what helps, improves.

With more and more peritherapeutic approaches emerging, such as coaching, mindfulness, yogatherapy and what not, psychologist become the philosophers and methodologists of psychotherapy . It is their job to understand the mind, to understand well being. To define goals for any  experimental approach.

Talking therapy would have its tradition, its great thinkers, and an advantage in some circumstances. But it would not be the face of psychotherapy.

Once we have established a way to measure results, we can experiment, be creative, find new metaphors, go by our fancy, and then see if it works. It doesn't matter what you do , as long as you  track results and improve your practices accordingly. A more humanistic, subjective way to go about it is necessary still, to avoid technocracy, disformations by concretization etc. Of course psychoanalytics is a tool to  be used in ethical hands.  But it is unethical not to use it.

Gotta Have a Freudian Quote for Psychoanalytics:

Freud's, A Brief Account of Psychoanalysis: Analytic Technique (1938):

"We would not be disappointed, and even would see it as obvious, that we may come to the conclusion that the final result of the battle we took to fight, relies on quantitative relations,  on the amount of energy that we can recruit to our favor with the patient in comparison to the amount of energy of the forces working against us. Here too, God is with the stronger legions; certainly we do not always win, but in most cases we can at least understand why we did not win."

Benjamin's Intersubjective View of Separation-Individuation

In her article Recognition and Destruction: An Outline of Intersubjectivity (1995), Jessica Benjamin presents the legacy of intra-psychic theory as having a "tendency to collapse other subjects into the rubric objects" and sets the relational task to change theory and practice to integrate intersubjective perspective: "where object were, subjects must be." (p1).

Benjamin wishes to discuss the experience of the other as object and as subject without contradiction, but marks the importance in the difference. "Intersubjective theory [in philosophy] postulates that the other must be recognized as another subject in order for the self to fully experience his or her subjectivity in the other's presence."(p2). Benjamin suggests that the other was seen as object as the first other is the mother, and women being seen as objects by western tradition made it hard for theorists to escape this idea. Instead, her main claim is, that the recognition of the mother as a subject by the infant, is a primary developmental task for the infant. This capacity develops unevenly, and eventually allows for mutual recognition between subjects.

Benjamin takes to examine Mahler's separation-individuation theory, with insights provided by Winnicott and Stern, and the added value of an intersubjective perspective on the process they describe. The theory describes the residue of the mother's interaction as an internalized object in the infant. This infantocentric approach, Benjamin claims, neglects "the engagement, connection, and active assertion that occur with the mother as other. unconcerned with the source of the mother's responses, which reflect not only her pathology or health ('narcissistic' versus 'good enough') but also her necessarily independent subjectivity. It also misses the pleasure of the evolving relationship with a partner from whom one knows how to elicit a response but whose responses are not entirely predictable and assimilable to internal fantasy" (p3). The infant must also display responsiveness, empathy and concern to the mother, in order for her to care for the infant, and for the infant to grow to be a "good enough" parent one day. The Kohutian empathic errors of the mother are opportunities for the infant to learn about the other's feelings, to be able to give empathy to, not just receive it, to enjoy mutuality. "By our very enjoyment of the other's confirming response, we recognize her in return" (p3)

Benjamin sees the seeds of intersubjectivity in the baby's smile to the mother, before the full understanding of the other. She refers to Stern, that describes the later stage of intersubjective relatedness at the age of 9 moths: "the new thing is the sharing of the inner world. The infant begins to checkout how the parent feels when the infant is discovering a new toy, and the parent demonstrates attunement by responding in another medium." In contrast, Benjamin refers to Mahler's Refueling, where "the checking back to look at mother is not about sharing the experience but about safety/anxiety issues" (p4).

Benjamin examines Mahler's rapprochement stage by Heggelian terms, as a tension between self and other: "in its encounter with the other, the self wishes to affirm its absolute independence [meaning, everything is me and I control everything], even though its need for the other [to recognize its subjectivity] and the other's similar wish undercut that affirmation." (p4). She describes the conflict of the self: "at the very moment we come to understand the meaning of I, myself, we are forced to see the limitations of that self".

In rapprochement stage, the infant is "able to do more.. [but becomes] aware of what she can't do and what she can't make mother do" (p5), so the infant focuses on the mother's independent aims, and the possibility that she will leave when the infant doesn't want her to go. The infant becomes tyrannical, undermining her separateness and her confidence in the infant's ability to survive it. If Mahler sees resolution of this intra-psychic tension by internalization of a balance between assertion self and recognition of other, then the intersubjective perspective would set a higher expectation, to recognize mother as a subject. To describe this task, Benjamin refers to Winnicott's idea of destroying the object: "If [mother] survives without retaliating or withdrawing under the attack, then we know her to exist outside ourselves, not just as our mental product" (p6). By surviving the destructiveness of the infant, the mother restores the good object, by Winnicott, and also, through an intersubjective perspective, it allows love, the discovery of the other. When the other does not survive the attack, aggression stays intra-psychic only. According to Benjamin, the tension between intrapsychic internalization and intersubjective reality is in the center of Winnicott's idea: " it is in contrast to the fantasy of destruction that the reality of survival is so satisfying and authentic" (p6). The infant's motivation changes then, from separation to be one and alone, to separation in order to be together: "To the extent that mother herself is placed outside, she can be loved" (p6). The developmental task that can be thus achieved is the establishment of a shared reality.

The infant evolves from a controlling retaliatory stance – I will do to you as you do to me, to a shared experience position – I understand why you wish to tend to yourself as I wish to tend to myself. The child reacts to recognition of the other with reluctance: "I insist on my way, I refuse to recognize you, I begin to try to coerce you; and therefore I experience your refusal as a reversal: you are coercing me" (p7). Later, around 3 years of age, the infant develops a more complex intersubjective capacity that alleviates this tension: "'You know what I feel, even when I want or feel the opposite of what you want or feel.' This advance in differentiation means that 'We can share feelings without my fearing that my feelings are simply your feelings'" (p7). This is an evolution from a power struggle between complementary roles of mother and child, subject and object, to a mutual understanding between two subjects.

Intra-psychic internalization of the object, is then seen as unachieved mutuality with the other. Complementarity is a step on the way to mutuality, and it makes intra-psychic theories valid to describe this stage of development. (p8). The mother's existence as subject as well as object, allows the infant to play with her as an object, without threatening the infant's subjectivity by destroying the object, as she still survives as a subject. This is the same in transference play, notes Benjamin. The ongoing

interplay of destruction of the object in fantasy and recognition of its separate existence in reality is a dialectic between fantasy and external reality.

Benjamin summarizes by giving place to destruction of the object. Mutuality is not an ideal harmony, she notes, but a constant breakdown and repair, a dialectic movement between using each other and sharing with each other.

My Comments

1) I found it to be a narcisstic axiom that one comes on the expense of the other, Benjamin elucidates this dynamic as retaliatory complementary perspective. The answer then would be, let's share experiences.

2) Benjamin's invitation to destroy is realistic. It accepts aggression and destruction motivation that exist in us. The solution is not to resolve these, but to relate to them.

3) Psychotherapy based on objective results, could be seen as the opposite of intersubjectivity. In my view, however, intersubjectivity could be the basis to understand collaborative psychoanalytics - using therapy data from different patient-therapist dyads and from different sessions, to compare and learn about the here and now in a therapy session. This would be more in the line of inter-objectivity, and allow the intersubjective field a richer basis to fall upon.

Psycho Analytics - Applying CRM Technologies in eHealth

The Gap
Currently, there are three fields of psychotherapy that are advancing in this direction, not yet capitalizing on data mining technologies. A brief description:
  1. Online Therapy: a variety of clinics using video chats and browser based tutorials to reach remote patients. The US Army is investing largely in this field, in order to provide psychotherapy solutions for overseas troops.
  2. eHealth / TeleHealth: Large healthcare organizations are developing online medical advice services, not specific to mental health, in order to reduce clinic occupancy and reach remote patients.
  3. Evidence Based Psychotherapy: a group of psycho therapy approaches, which are supported by a lot of academic research, and show measurable effect on symptoms, such as cognitive behavioral therapy (CBT). These approaches are usually short term, and low cost compared to long term approaches such as psychoanalysis, so they are championed by insurance companies and included in healthcare policies.
  4. Bio Feedback Therapy: an approach with inherent measurement tools (such as heart rate) that has adapted the most to online platforms and the evidence based approach. Bio feedback technologies are rich in data, benefit from analytic technologies and make good use of mobile platforms. They are as popular as an over the counter iphone app. US Military, NASA, and insurance companies prefer it when possible. However, it helps only patients with physical arousal as main negative symptoms.
An approach which is obviously missing in this list is the commonly known psychodynamic therapy, even though it remains the preferred therapy by therapists for the majority of cases. Many other approaches are waiting for academic prestige before they can even be tested.

The Potential
People spend fortunes so that someone will listen to their story and analyze their words in order to feel better. Psychotherapists listen a lot, compare to theories and research done on small samples and analyze this data, manually. There are relationships and inspirational effects, but systematic data analysis is a main tool.
Current CRM technology could be suited to analyze therapy sessions, as recorded in clinics or online. Current measures of biofeedback, self appraisal, and analyst appraisal would serve as desired results.
Using collaborated data analytics to manage psychotherapy process toward results, could help patients get what the best for their investment and for their problem, expose more therapy approaches to testing, and gain financial support by large insurance organizations.

Is It Alright to Touch You?

This semester I've been guiding a gender conflict group. A gender conflict group, as I see it, is a group of people gathered together to raise awareness to their gender role in society, to investigate their positions and feelings toward their gender role, and, most importantly, to act out and experience a complex inner discussion through a group discussion that brings up many different voices that represent silenced inner voices.

A common discussion in these groups, that also took place in the group I participated in last semester, is about rules that regulate the relations between men and women. For example, what is definitely wrong for a man to do to a woman, such as physical violent acts, what is OK, such as smiling and saying hi, and what is unclear, such as touching, as in some contexts this is OK, while in other contexts this is definitely wrong.

Saying that this depends on context is not completely helpful, as some contexts for touching are clear, such as consensual intimate lovers who can touch and it would be clearly OK, or strangers in the streets who would be clearly wrong to touch. But what about contexts that do not determine things so clearly, such as a romantic date, where people are no longer strangers but not yet consensual intimate lovers?

Well, if the context is not clearly wrong, you should just ask, is it alright to touch you? If yes - yes, if no - no, if insulted by the question - reconsider your list of wrong contexts.

This discussion could end with this simplistic algorithm, trying to optimize petitions of consent. I think it feels weird, and although it is very politically correct, we know that this doesn't face the deeper complexity. It could be criticized as objectification of women, as touch-things, that can only express yes/no feedback to their function as objects of desire, and not a more complex subjective position. Men are also more than desiring objects...

So what would be a more complex subjective position to objectification? Well, it's hard to say, but I think we should consider how good it feels sometimes to be an object of desire, and how the same feeling could become horrible in its disrespect, to see that there is a deeper dynamic of subject-object within us. I will read some more in the direction of Kristeva and Zizek before I can truly explain what I mean. Anyway, to make this post add good to the world, as a rough thumb rule, a respectful question in unclear situations could help.

Psychotherapy Behind a One-Way Mirror

After a semester of watching psychotherapy sessions behind a one-way mirror, I feel like this has been the best qualification method I have experienced so far. During my training, I've read and heard vignettes, I have watched instructional recordings and watched TV shows about psychotherapy, I've done simulations, I have even done therapy myself, and I have brought verbatim transcripts of them to mentoring sessions. Nothing prepared me as well as watching a therapist in real time, connecting to the patient and the therapist in direct and indirect means, feeling the space between them, second guessing the therapist, shutting my eyes and listening without having to do anything, trying to assume and identify with both positions in the sessions at different moments of every session. I feel this was a very full and rich experience, which allowed me both the safety of distance from the situation, and the vivid vitality of connecting to it myself. Other training modes were either too partials, teaching technique from theory, or too flooding, not letting me process and reflect the finer nuances that fill the therapy room.

I think it is not by chance that I did not have similar experiences. Psychotherapy has not been taught to me as a craft. It has been passed to me part as science, with academic learning of theories, and part as art, with personal mentoring after experience. To teach a craft one must assume that there is A Way to Do It, a technique, and that there are skills and intuitions that must be acquired for it to be Done Well. Mimicking a skilled therapist from behind a mirror taught me psychotherapy as a craft.

Watching Prof. Dar, specifically was very interesting, as he has a very commonsensical, experience-near approach to therapy. With the patient, it seemed there are no theoretical obstacles or efforts to agree with conceptualizations against what is experienced. With the class, in after-session discussions when we students suggested fancy psychoanalytic concepts, Dar had a very clear measure of what is relevant or irrelevant to the patient experience. Psychodynamic concepts were still very helpful in the post-session discussion, when we guessed where the patient could be leading the process in future sessions and prepared for it, but these did not lead the process by themselves.

Micro Horses, My Thesis, and Unicorns

I have a hard time giving up on my idea to analyze therapy sessions with a voice recognition interaction analytics CRM system.

Why give it up? Because it's taking too long and I usually have these high hopes that are unreal and maybe this is one too. The researcher who holds the recordings is not too keen on helping me on my sidetracking quest. And I need a thesis already. I could write an OCD themed thesis very easily. But I want to change the world. Perhaps I am obsessed with it.

So where does it stand at the moment? I need to get the university's ethics commity to approve my procedure, and the guy in charge of running it to sign a secrecy agreement. Not too far fetched.

So why is it hard? Coz I got over excited about the meaning of the results.

I think the micro horse teaches us that unicorns might also exist, and that fairy tales should not be rejected by the scientific mind.  Also, it may teach us that even a small horse needs a stable, and that a small thesis is not much easier to overcome than a large one in terms of procrastination overhead.

PS 14/06/12 :

Since after 3 months, this post has proven to be the leading clicker here, and it has little content really, and micro horses are ruining the SEO for psychoanalytics, I have decided to go with what  works, as this is the essence of psychoanalytics: starting today,  I declare micro horses as the icon for psychoanalytics. There is a Biblical saying about serendipity: Saul went out to look for his father's horses and found the royalty of Israel. In short Sought mules, found kingdom. I think this is the idea behind psychoanalytics - being set to find the unexpected.

Fanning the Flame

Maybe it's obvious, but I see people, top professionals overlooking this question, as if it's just about gut feeling and experience.

For a few years now I have been taking part in therapist working groups in various forums. In these, a case is for consultation in confidence by a therapist, and a group of colleagues provide additional  insight to that of the therapist. It's a great thing to do for many reasons that are not the issue of this post.

Often, a patient is described, for example, let's imagine a patient that is dealing with a fear of flight. The therapists think, analyze the meaning of this fear, and reach a rich description of how it probably feels like to be the patient, and what deep meaning every common thing bears for the patient. For example, the fear is related to that patient's need of control. This is very easy for therapists to achieve, as it requires a simple summation of their most creative associations.

Then sometimes comes a question: so what to do about it? We understand that the patient wishes for, or even needs control - do we give him (could be her, sure) some sort of control, or do we make him give up control? We must see that both are possible, and both could be good. Challenging a patient out of his comfort zone is good. Rushing a patient out of his safety zone is bad. Many discuss these as matters of faith and personal taste. That makes them right about half of their decisions, and blind about the factor that makes them right or wrong.

So, to my view, and I am very surprised at this being an overlooked issue, the issue is ego powers. I see a person's mental powers as a sinusoidal wave, with ups and downs over time. A person could meet a difficulty when he's down, and that would weaken and deplete his ego (mental capacities) even further, or, he can meet  a difficulty when he's strong, and that would forge him even stronger. The same difficulty could ruin or build. Challenging a patient should then be considered according to his mental capacity to build from that as a challenge and not weaken from it as a handicapping intervention.

People need challenges, to grow, to be able to experience the world, with its excitement and hardships. Being a nice and facilitating a way around an obstacle isn't serving the patients' needs. Help could be handicapping too, if it does not acknowledge the capacity of the patient to take on challenge.

This changes over time for every person. In the insightful description of Stephan Mitchell of how kids grow their capacity to deal with the world through their parents' mediation, he describes a child that ask for juice at the age of two. The parent applause the child, and provides the juice. At the age of 3 that child, a verbose and industrious creature already, asks for juice, and is suddenly asked to say please or even get it himself (or herself, of course). We keep growing, and our expectations of ourselves need to keep advancing a bit ahead of where we are. Just a bit.

My metaphor for this is of the Israeli MANGAL. The charcoal grill. We light it with fire, then we fan the flames with wind. If you fan it too soon, you turn the fire out, if you fan it too late, you lose the fire's grip on the charcoal. To fan it in time is the work of the master MANGALIST. The same is for challenge in therapy.

PS - I apologize for either writing the obvious, or saying that it's obvious.

PS (10.9.2012): I think this makes more sense talking about agency. And I also think Winnicott prethought of it.
